Job Description
We are looking for a dedicated Registered Nurse (RN) Field Nurse with experience in Home Health Care to join our team at ChristianaCare. As an RN in our Home Health division, you will provide expert care to geriatric patients in the community, develop care plans, and ensure patient-centered treatment. Responsibilities
- Deliver skilled nursing care and treatments to geriatric patients in the community.
- Develop and implement care plans aligned with patient goals and available resources.
- Prepare patients for discharge and ensure continuity of care.
Requirements
- BSN required; ADN candidates must obtain a BSN within three years of employment.
- Current DE RN license or compact state license.
- Minimum 2 years of recent Home Health RN experience.
- Proficiency in IV therapy or willingness to train.
