Job Description

We are looking for dedicated and experienced Licensed Practical Nurses (LPNs) to join our HomeHealth team at ChristianaCare. This is an exciting opportunity to provide skilled nursing care to geriatric patients in the community, working in a supportive environment with comprehensive OASIS-E training and advanced documentation tools like Matrix Care.

Responsibilities

• Provide skilled nursing care and treatments to geriatric patients in the community.
• Implement patient-centered care plans.
• Participate in developing discharge plans for patients.

Requirements

• High School Diploma (HSD) or GED.
• Current DE LPN license or compact state license.

Job Types

Per diem, Part-time, Full-time

Pay

$22.00 - $28.00 per hour

20 - 40 per week
