Job Description

Liberty Hospice, a leading provider of compassionate end-of-life care, is currently seeking a dedicated Hospice Medical Social Worker to join our team in Cumberland County on a full-time basis. As a Hospice Medical Social Worker, you will play a crucial role in providing direct patient care services, conducting clinical assessments, developing care plans, and offering support to patients and families during challenging times.

Responsibilities

  • Provide direct patient care services and maintain accurate documentation.
  • Conduct clinical assessments to create individualized care plans and treatment goals.
  • Apply psycho-social principles to evaluate social, emotional, and environmental factors influencing patient care.
  • Collaborate with agency personnel and coordinate services to optimize patient care.
  • Offer bereavement support and grief care to patients and caregivers.

Requirements

  • Master's degree in Social Work from an accredited institution.
  • Minimum of one year of practical experience, preferably in home health/hospice care.
  • Strong interpersonal skills and ability to communicate effectively in a multidisciplinary environment.
  • Willingness to travel and work under assigned conditions.
  • Maintain at least twelve hours of continuing education annually.
