Job Description
We are looking for a dedicated Registered Nurse - Intake & Hospital Liaison to join our team at UVM Health Network Home Health & Hospice. In this crucial role, you will be at the forefront of our operations, facilitating the seamless transition of patients to home health and hospice services. As part of our Intake and Referral team, you will collaborate with referring providers, program leaders, clinicians, and patients/families to ensure timely and effective care delivery.
Responsibilities
- Act as the first point of contact to support patients and referring providers.
- Provide education and information on services offered by the organization.
- Attend rounds, coordinate patient discharges, and review clinical documentation.
- Determine patient clinical eligibility for services and write orders for care.
Requirements
- Associate's degree in nursing and RN license.
- Minimum of two years of RN experience.
