Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD, specifically at Levindale Hospital. As a Social Worker, you will provide essential clinical social work services and psychosocial assessments to patients and families, addressing a range of common challenges such as adjustment to institutional living, financial concerns, family dynamics, medical understanding, aging-related issues, discharge planning, and bereavement support.

Responsibilities

• Deliver clinical social work services and psychosocial assessments to patients and families.
• Address various challenges including financial concerns, bereavement, discharge planning, and more.
• Collaborate with interdisciplinary teams to ensure comprehensive care.

Requirements

• Bachelor’s degree with 1-3 years of experience.
• LBSW/LMSW/LGPC certification.
• Strong interpersonal and communication skills.
