SOCIAL WORKER PRN

Job Description

We are currently looking for a PRN Social Worker to join our compassionate team at LifeBridge Health in Baltimore, MD. In this role, you will provide essential clinical social work services and psychosocial assessments to patients and their families, covering a range of common issues related to institutional living, financial concerns, aging, discharge planning, and more.

Responsibilities

• Conduct psychosocial assessments and provide clinical social work services.
• Address various patient and family concerns including adjustment to institutional living, financial matters, and end-of-life issues.
• Collaborate with healthcare professionals to ensure comprehensive care planning.

Requirements

• Bachelor's degree with 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
• Strong understanding of social work theory and practice.
