SOCIAL WORKER PRN

Job Description

We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. As part of the social work-hospital team at Levindale Hospital, you will provide clinical social work services, conduct psychosocial assessments, and offer support to patients/residents and their families facing various challenges.

Responsibilities

• Deliver psychosocial assessments to patients and families.
• Address common problems such as adjustment to institutional living, financial concerns, and family relationships.
• Provide counseling on medical issues, aging, discharge planning, and end-of-life care.

Requirements

• Bachelor's degree and LBSW/LMSW/LGPC certification.
• 1-3 years of relevant experience in social work.
• Strong professional knowledge and understanding of social work practices.
