Job Description

We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health in Baltimore, Maryland. As a Social Worker, you will be responsible for providing clinical social work services and psychosocial assessments to patients and their families, addressing various challenges related to institutional living, financial concerns, family relationships, medical procedures, aging, discharge planning, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments of patients and families.
• Provide counseling and support related to adjustment to institutional living.
• Assist with financial concerns and discharge planning.
• Offer counseling on end-of-life issues and grief support.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed Social Worker (LBSW/LMSW/LGPC).
