Job Description
We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges such as adjustment difficulties, financial concerns, family relationships, discharge planning, and counseling related to death and dying.
Responsibilities
- Conduct psychosocial assessments and offer counseling services.
- Assist patients with issues related to aging, family relationships, and institutional living.
- Collaborate with healthcare professionals to provide comprehensive care.
- Participate in discharge planning and support patients in end-of-life situations.
Requirements
- Bachelor's degree in Social Work or related field.
- 1-3 years of relevant experience.
- Licensed as LBSW, LMSW, or LGPC.
- Knowledge of social work theory and practice.
