Responsibilities
- Conduct psychosocial assessments.

- Collaborate with healthcare teams on discharge planning.
Requirements
- Bachelor's degree in Social Work or related field.
- 1-3 years of relevant experience.
- Licensed as LBSW, LMSW, or LGPC.
- Strong interpersonal and communication skills.
