Job Description

We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and their families, addressing a range of common issues such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.

Responsibilities

• Provide clinical social work services and psychosocial assessments to patients/residents and families.
• Address common issues including adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.
• Participate in discharge planning and provide support related to aging and powerlessness.

Requirements

• Bachelor's degree and relevant professional knowledge.
• 1-3 years of experience as a Social Worker.
• LBSW/LMSW/LGPC certification.
