Job Description
We are looking for a compassionate Social Worker to join our team at LifeBridge Health. As a PRN Social Worker at Levindale Hospital in Baltimore, MD, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families. Your primary focus will be on addressing common issues such as adjustment to institutional living, family relationships, financial concerns, aging-related changes, and counseling related to death and dying.
Responsibilities
- Conduct psychosocial assessments.
- Provide counseling and support to patients and families.
- Assist with discharge planning.
- Collaborate with healthcare teams to address patient needs.
Requirements
- Bachelor's degree in Social Work or related field.
- 1-3 years of relevant experience.
- LBSW, LMSW, or LGPC certification.
- Strong interpersonal and communication skills.
