Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a part of our esteemed medical institution, you will provide essential clinical social work services and psychosocial assessments to patients and their families, addressing a range of common challenges such as adjustment to institutional living, financial issues, family dynamics, and end-of-life counseling.

Responsibilities

• Deliver clinical social work services and psychosocial assessments to patients and families.
• Address a variety of issues including adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.
• Assist with discharge planning and counseling related to aging and death.

Requirements

• Bachelor's degree and LBSW/LMSW/LGPC certification.
• 1-3 years of relevant experience.
