SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. The position involves providing clinical social work services and psychosocial assessments to patients/residents and their families. You will address various challenges such as adjustment to institutional living, financial concerns, family relationships, discharge planning, and counseling for end-of-life matters.

Responsibilities

• Provide clinical social work services to patients/residents and families.
• Conduct psychosocial assessments and address a range of common issues.
• Assist patients with adjustment to healthcare procedures and treatments.
• Collaborate with interdisciplinary teams to support patient care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
• Strong understanding of social work theory and practices.
