Job Description
We are looking for a dedicated and compassionate PRN Social Worker to join the LifeBridge Health team at Levindale Hospital in Baltimore, MD. The ideal candidate will provide clinical social work services to patients/residents and families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life care.
Responsibilities
- Conduct psychosocial assessments and provide counseling to address patients' social and emotional needs.
- Assist with discharge planning and facilitate transitions to post-hospital care settings.
- Collaborate with interdisciplinary teams to ensure comprehensive care for patients.
Requirements
- Bachelor's degree and 1-3 years of relevant experience.
- LBSW/LMSW/LGPC certification.
- Strong communication and empathy skills.
- Ability to work effectively in a fast-paced healthcare environment.
