SOCIAL WORKER PRN

Job Description

We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. The ideal candidate will provide clinical social work services and psychosocial assessments to patients and their families, addressing various psychological and social challenges that may arise during their stay.

Responsibilities

• Offer support and counseling to patients and families on adjustment to institutional living, financial concerns, and family relationships.
• Assist individuals in understanding and coping with medical diagnoses, procedures, and treatments.
• Facilitate discharge planning and provide counseling on end-of-life issues.

Requirements

• Bachelor's degree in social work.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
