Job Description
We are looking for a dedicated Social Worker to join our team at LifeBridge Health. In this role, you will provide professional, comprehensive, and family-oriented services to individuals in their chosen residence. The ideal candidate will assess psychosocial, environmental, and financial needs of patients and families, collaborate with interdisciplinary teams, and ensure high-quality care delivery.
Responsibilities
- Assess patients' and families' psychosocial, environmental, and financial needs.
- Formulate and implement personalized care plans in collaboration with patients, families, and interdisciplinary teams.
- Act as a consultant, educator, and expert practitioner within your area of expertise.
- Ensure evidence-based practice and optimal patient/family care.
Requirements
- Master's Degree in Social Work from an accredited school.
- LCSW certification required.
- Excellent interpersonal communication skills.
- Ability to maintain accurate documentation records.
