Job Description
We are looking for a dedicated Social Worker to join LifeBridge Health, one of Maryland's largest healthcare providers. In this full-time role at Levindale Hospital in Baltimore, MD, you will provide clinical social work services and psychosocial assessments to patients/residents and their families. Your responsibilities include addressing a variety of social, emotional, and financial concerns that patients may encounter during their treatment and recovery process. If you have a Bachelor's in Social Work (BSW), with a preference for a Master's degree and LCSW-C certification, and possess a compassionate approach to patient care, we encourage you to apply.
Key Responsibilities
- Provide counseling and support to patients and families facing challenges related to medical conditions, aging, and end-of-life care.
- Assess and address social and financial barriers impacting patients' well-being.
- Collaborate with healthcare professionals to develop comprehensive care plans and discharge strategies.
- Contribute to the multidisciplinary team to ensure patient needs are met holistically.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree and LCSW-C certification preferred.
- Proven experience in clinical social work services with a focus on aging, palliative care, and mental health.
- Strong communication, empathy, and organizational skills.
- Ability to work effectively in a fast-paced healthcare environment.
