Job Description
We are looking for a compassionate Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and their families, addressing a variety of concerns related to institutional living, financial issues, relationships, medical procedures, aging, and other challenges.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and their families.
- Offer support and guidance on issues such as discharge planning and end-of-life care.
- Collaborate with healthcare teams to ensure holistic patient care.
Requirements
- Bachelor's in Social Work (BSW) required, Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW).
