Job Description
We are currently seeking a passionate Social Worker to join our dedicated team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and their families, addressing a wide range of emotional and social challenges they may face.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and families.
- Assist individuals with adjustment to institutional living, financial concerns, and family relationships.
- Collaborate with medical teams to address emotional and psychological challenges related to diagnoses, treatments, and aging.
- Support patients and families with discharge planning and end-of-life counseling.
Requirements
- Bachelor's Degree in Social Work (BSW) required, Master's degree preferred.
- Licensed Certified Social Worker - Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
