Job Description
We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges they may face during their healthcare journey. The position is full-time, day shift, based in Baltimore, MD, at Levindale Hospital.
Responsibilities
- Offer clinical social work services to individuals and families facing various issues.
- Assess common problems such as adjustment difficulties, financial concerns, family dynamics, etc.
- Provide counseling and support related to aging, powerlessness, and end-of-life planning.
- Collaborate with healthcare teams for effective patient care and discharge planning.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree is preferred.
- LCSW-C certification preferred; LGSW certification required.
- Prior experience in a healthcare setting is a plus.
