Job Description
We are looking for a compassionate Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and families, addressing various common challenges such as adjustment to institutional living, financial concerns, family relationships, and more. Responsibilities
- Provide psychosocial assessments to patients and families.
- Offer counseling related to various emotional and social challenges.
- Assist with discharge planning and end-of-life care.
- Collaborate with other healthcare professionals to ensure comprehensive patient care.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- LCSW-C certification preferred; LGSW license required.
- Prior experience in clinical social work is a plus.
- Strong communication and interpersonal skills.
