Job Description
We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will be responsible for providing clinical social work services and psychosocial assessments to patients/residents and families, addressing a range of issues such as adjustment to institutional living, financial concerns, family relationships, and discharge planning.
Responsibilities
- Provide clinical social work services and psychosocial assessments to patients and their families.
- Address common problems related to institutional living, family dynamics, and end-of-life counseling.
- Collaborate with interdisciplinary teams to develop comprehensive care plans.
- Assist in discharge planning and ensure continuity of care.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW).
- Compassionate and empathetic approach to patient care.
