Job Description
We are looking for a compassionate Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, offering support and guidance on a range of challenges they may face. Responsibilities
- Provide psychosocial assessments and support to patients and their families.
- Address common issues such as adjustment difficulties, financial concerns, and discharge planning.
- Offer counseling and assistance with end-of-life discussions.
Requirements
- Bachelor's in Social Work (BSW); Master's degree preferred.
- LCSW-C preferred; LGSW certification.
- Excellent communication and interpersonal skills.
