Job Description
We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health. In this full-time position, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges they may face. You will play a vital role in supporting individuals through difficult times, including adjustment to institutional living, financial concerns, family relationships, and end-of-life issues.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and families.
- Assist with discharge planning and end-of-life care.
- Collaborate with healthcare teams to address patients' social and emotional needs.
- Advocate for patients' rights and access to resources.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- LCSW-C certification preferred.
- LGSW certification required.
- Compassionate and empathetic approach to patient care.
