Job Description
We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a variety of common issues such as adjustment to institutional living, financial concerns, family relationships, and end-of-life care.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and families.
- Assist with discharge planning and offer support related to aging and end-of-life decisions.
- Collaborate with healthcare teams to address patients' emotional and social needs.
Requirements
- Bachelor's in Social Work (BSW) required, Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
