Job Description
We are looking for a passionate Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families, addressing a range of emotional and practical challenges. Your role will involve offering support related to institutional living, financial issues, family dynamics, medical treatments, aging, and end-of-life care.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and families.
- Assist with discharge planning and coordinate post-discharge care services.
- Advocate for patients' needs and collaborate with healthcare teams to ensure comprehensive care.
- Provide emotional support and coping strategies for patients and families facing challenging circumstances.
Requirements
- Bachelor's in Social Work (BSW) required, Master's degree preferred.
- LCSW-C certification preferred.
- LGSW certification required.
- Strong communication and interpersonal skills.
