Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

Genesis Health is looking for a diligent Minimal Data Set Coordinator who is a Licensed Practical Nurse. As an LPN MDS Coordinator, you will play a vital role in the Resident Assessment Process by completing necessary items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure proper patient care and nursing center reimbursement.

Position Highlights

• Lead the nursing center interdisciplinary RAI process in compliance with guidelines.
• Create individualized resident Care Plans based on resident needs and goals.
• Train new team members on clinical content and MDS completion.

Qualifications

• Active LPN license in good standing.
• Prior experience in hospital or long-term care settings.
• Familiarity with nursing practices and regulations.
• Proficiency in computer applications, preferably with AccuMed experience.
• Previous MDS Coordinator experience is a plus.
• Ability to maintain BLS/CPR certification.
