Job Description
We are looking for a dedicated Complex Care Social Worker – LMSW to join our team at LifeBridge Health. In this full-time position based in Baltimore, MD, at Sinai Hospital, you will play a vital role in managing discharge planning for challenging cases to ensure efficient and cost-effective patient care.
Responsibilities
- Utilize a multi-step escalation protocol to identify patients with barriers to timely discharge.
- Collaborate with the interdisciplinary team to facilitate effective discharge planning.
- Monitor difficult-to-place and high-cost patients for early intervention opportunities.
- Adhere to set standards and utilization review guidelines for discharge planning.
Requirements
- Master's degree in Social Work.
- Licensed Master Social Worker (LMSW).
- Seasoned professional knowledge and expertise in the field.
